Non-National Health Service Requests, including referrals from outside the UK
As reports for non-NHS requests will be issued by encrypted email, it is essential to provide us with an email address to which the report should be sent.

In addition to the standard charge for each test (see relevant disorder-specific page, or click here for full list of tests and charges) there is a 25% overhead charge for all non-NHS requests.
Please complete our form with details of where the invoice for the test is to be sent and send it to us with the sample. Failure to return this form will result in a delay in testing.
