Learning Disability Liaison Service - For Staff

The Learning Disability Liaison Team are available Monday to Friday 8am – 4pm. 01392 402237. When we are not in the office and outside of these hours you can leave a message which we will respond to as soon as able.

 

We also have a team Email - rde-tr.learningdisability@nhs.net. Please refer all adult patients with a learning disability to the Learning Disability Liaison team.

 

Definition of a Learning Disability

 

Adults will be considered to have a Learning Disability if there is :

Hospital Passport

 

All patients with a Learning Disability should have a hospital passport which has been completed in the community. If one is not available complete one with the person and or their carers. Ask us to support if needed.

 

This should be kept with the nurses notes throughout admission and then returned to the patient on discharge.
