Please phone the number in your appointment letter as soon as you can to tell us if you cannot come on the date or time we have given you.
Phone us as well on the same number to tell us about your special requirements, such as you are disabled, have mobility problems, use special equipment, (such as a hoist, support chair or pressure relieving devices) or have pressure ulcers.
If we ask you to bring a urine sample, please bring this in a plastic container. You might be able to get this from your GP surgery.
