
EMERGENCY INFORMATION SCHEME
THIS COULD SAVE A LIFE – YOUR LIFE!
- Your vital information will include your Personal details, Doctors name, Medical conditions, Allergies, Care/Health visitor details or dependents and Family contact details.
- Repeat prescriptions may also be stored in the bottle for current medication information.
- “Message in a Bottle” can save lives, if nothing else it will save the Emergency Services valuable time and money searching for vital information.

How to use the scheme
1. Complete the form and put in the bottle.
2. Place the bottle in the fridge.
3. Place one green sticker on the inside of the front door (of your home).
4. Place the other on the outside of the refrigerator.
All the Emergency Services are aware of the scheme and will use this vital life saving information in the event of an emergency.
BOTTLES ARE AVAILABLE FREE OF CHARGE
from your Local Lions Clubs, most GP Surgeries and Pharmacies
