Protection and use of information about patients

In order for us to provide you with high quality care and treatment we need you to provide us with up to date information.

 

All of our staff at the Trust have a legal duty to keep all your information confidential.

 

Why we need your information:

 Information about you and your care are retained in case we ever need to see you again and some of these details may be kept electronically.

 

By choosing not to share this information it could mean that we would be unable to provide you with the best and most appropriate treatment or care.

 

Having this information can give us a greater understanding of our patients and their needs. The NHS can then plan and develop services to meet these needs.

The information you provide us could also be used to help educate future clinical staff, or assist medical or health research.

 

If you are receiving care from other healthcare services, as well as the NHS, we may need to share some of your information with them. This enables us all to provide the best care we possibly can.

 

Information is only ever shared when there is a genuine need for it and whenever possible we will remove details which identify you.

Those who receive information from us are under the same legal duty to keep it confidential as our own staff

 

There are times when we are required by law to pass on information – for example, to notify the birth of your child.

If you wish, your relatives, friends and carers can also be kept up to date with the progress of your treatment.

 

The NHS Central Register for England & Wales contains basic personal details of all patients registered with a general practitioner. However, the register does not contain any clinical information.

 

You can access any information that the Trust holds on you – in paper or electronic form – under the Data Protection Act 1998. A charge may be made for this service.
